It's Jane's Birthday, Maura and Angela are setting up a surprise birthday party but Maura can't find out what to get her. But when a prisoner inmate is murdered, Jane and Maura discover that Hoyt is in the prisoner infirmary. Hoyt is receiving treatment for cancer, and he has offered to give Jane information on murders that he has done years ago. Hoyt gives Jane and Maura the clues, but he said it in a riddle. Maura believes that the cancer has spread to his brain and he doesn't know what he's saying, but Jane believes and wants to give the family some peace. Maura finds a balloon full of four teeth in the victims intestines. Jane finds out that a family of four had gone missing in 2005, at the same college their victim had gotten accepted to, it is also the same college that Hoyt got kicked out of years ago. Jane tells Maura that she doesn't like her birthday because she doesn't want to get disappointed. When Hoyt wants to see Jane and give up everything on everyone he's killed, Maura goes with her, and he attacks Jane, while a guard holds Maura. Korsak and Frost find the guards prints on the bail money for their victim. When Hoyt goes after Maura, Jane attacks him and kills him with a scapel.
